Late Corneal Decompensation in Congenital Glaucoma with Extensive Haab Striae.

A 28-year-old woman with a history of congenital glaucoma presented with a 2-year history of progressive blurred vision in both eyes. Visual acuity was 20/400 in the right eye and 20/100 in the left eye. On slit-lamp examination, we noted extensive Haab striae (A) and corneal edema. The patient underwent Descemet stripping automated endothelial keratoplasty in the left eye with improvement of visual acuity to 20/40 at 1 month after surgery (B). Patients with congenital glaucoma may have Haab striae that persists into adulthood, and the underlying corneal endothelium may decompensate, leading to corneal edema and vision loss (Magnified version of Fig A-B is available online at www.aaojournal.org).
