•Patients with NAFLD cirrhosis did not present a higher rate of liver-related events during the COVID-19 pandemic.•Usual predictors, such as diabetes, albumin and FIB-4 were associated with higher risk of a first liver event.•Health system preparedness seems key to ensure patients with NAFLD cirrhosis receive appropriate care during health crises.
