•Whereas the extent of RV structural remodeling is tightly linked to the degree of RV afterload, structural remodeling is dissociated from RHF•The transition of RV dysfunction to RHF is mediated by mitochondrial ROS•In contrast to 6N mice, 6J mice are protected from RHF upon RV pressure overload caused by reduced oxidative stress on account of genetic deficiency of the mitochondrial NNT•In patients with end-stage LHF, severely impaired RV function is associated with increased oxidative damage
