•Recipients with discordant donors show poor cytomegalovirus (CMV)-specific cell-mediated immunity (CMI) reconstitution after hematopoietic stem cell transplantation.•Impaired CMV-CMI reconstitution persists even after CMV reactivation.•Discordance in donors is associated with low frequency of CMV recurrence.•CMV-CMI analysis in donors might help classify the risk of poor reconstitution.
