•Hepatitis E IgM titres dichotomise around two well-separated modes.•Confirmatory tests after a reactive hepatitis E IgM confer minimal clinical benefit.•Immunocompromised patients should have IgM and PCR (or similar) tests.•Current literature is consistent with frequent early or rarer prolonged viraemia.
