•Bilateral retinal astrocytic hamartomas (RAH) may become symptomatic in adulthood.•FA and OCT are important diagnostic tests when evaluating symptomatic RAH.•RAH can generate subretinal fluid independent of choroidal neovascularization.•RAH-associated subretinal fluid may respond to anti-VEGF therapy.
