•People with epilepsy may die unexpectedly without a clear pathological cause for death; this is called SUDEP.•The incidence rate of SUDEP is 23 times that of sudden death in the population.•The most important risk factor for SUDEP is occurrence of tonic-clonic seizures.•Neurologists often do not discuss SUDEP with PWE and care-givers according to surveys.
