•ZnT8 autoantibody positivity (ZnT8+) is very low in youth with Type 2 diabetes.•ZnT8+ is associated with high insulin sensitivity and low insulin secretion.•All ZnT8 + participants exhibited loss of glycemic control.•ZnT8 + participants demonstrated low rates of diabetic complications.
