•Across 38 trials we identified 51 outcomes 119 measurement instruments applied.•We provided new knowledge on the heterogeneity of outcomes and instruments applied.•Existing core outcomes hardly include outcomes applied in self-management trials.•Further investigations into patient's perspective on relevant outcomes are needed.
