A 40-year-old female was referred for investigation of an organic cause of new psychosis manifesting as fluctuating disorientation, hallucinations, and altered speech, on a background 2 year history of PTSD and multiple psychiatric diagnoses including depression requiring ECT. The acute decline occurred in the context of treatment with clozapine, lithium and valproate. She reported drinking alcohol infrequently and denied illicit drug use.
