•There is a need for a validated scoring system to grade OA in the ankle using MRI.•An MR grading system for ankle OA was developed by an expert panel of clinicians.•Experienced observers can grade ankle OA on MR with high reproducibility.•MRI can be used for phenotyping ankle OA in prospective interventional studies.
