Cascade of care for office-based buprenorphine treatment in Bronx community clinics

•A cascade of care was constructed for all patients referred for office-based buprenorphine treatment.•Over a third of patients referred for treatment did not complete the initial visit.•Self-referred patients or those using nonprescribed buprenorphine appeared more likely to initiate office-based buprenorphine treatment.
