Disparities in patient and system factors explain racial/ethnic disparities in delayed time to treatment in muscle invasive bladder cancer

•Among patients with muscle invasive bladder cancer, those who identified as Black and Hispanic were more likely to have treatment delays compared to those who identified as White.•Black and Hispanic patients compared to White patients were more likely to be treated at academic hospitals, more likely to have lower education, and more likely to have Medicaid insurance which were all associated with treatment delays.•Mediation analyses suggested that 49% and 26% the treatment delay among Black and Hispanic patients, respectively, could be removed if an intervention equalized the distribution of academic treatment, education, and insurance status to that of White patients.
