Information exchange among providers and patient-centeredness in transitional care: A five-year retrospective analysis

•Considerable variation regarding transitional care program design, strategy selection and implementation occurred. Which combinations of TC strategies are most effective at improving patient outcomes remains to be elucidated.•When implemented together, information sharing across settings, engaging patients in education, and/or addressing patient needs were associated with the greatest reduction in readmissions.•Our analysis suggests that hospitals adopted TC groups preferentially based on baseline readmission rates and likelihood of HRRP penalties.
