•Foreign bodies may cause duodenal perforation and hepatic abscess formation.•Ingested pens may be the cause, but most commonly from bone fragments.•Presenting signs and symptoms may be non-specific (fever, pericardial effusion).•Computed tomography imaging is the workhorse in making this difficult diagnosis.•Most common definitive management is via laparotomy or percutaneous drainage.
