Before the door: Comparing factors affecting symptom onset to first medical contact for STEMI patients between a high and low-middle income country

•Early reperfusion in STEMI is associated with decreased morbidity and mortality.•Symptom onset to FMC consumes the majority of total ischemic time.•Contextual modifiable and non-modifiable factors affect delay time to care.•Modifiable factors include actions of bystander/patient, and transportation method.•Non-modifiable factors include gender, symptom type and severity.
