•A minor group of Iranian patients with MS has vaccine hesitancy.•Concerns about side effects was the most frequent reason of vaccine avoidance.•Young age, unemployment, and no academic degree were associated demographic factors.•Negative flu vaccination history was associated with hesitancy.
