THE ECONOMIC BURDEN OF THROMBOEMBOLIC EVENTS (TE) AMONG PATIENTS WITH INFLAMMATORY BOWEL DISEASE (IBD)

IBD is associated with higher risk of TE. This study estimated TE associated healthcare costs among patients with IBD. Adults with IBD who experienced TE (≥1 diagnosis for deep vein thrombosis [DVT], pulmonary embolism [PE], myocardial infarction [MI], and/or ischemic stroke [IS]) were identified from the IBM MarketScan Commercial and Medicare Supplemental Claims databases (2014-2018). Patients with IBD who experienced TE were matched to patients with IBD without history of TE on age, sex, and year of first IBD diagnosis. Index date (date of first TE following the first IBD diagnosis for patients with TE) was calculated based on time from first IBD diagnosis to index date of the matched case for patients without TE. Outcomes included all-cause and TE-related healthcare costs (cost of medical visits with TE diagnosis) in the 30 days and 1 year after the index date (inclusive). Unadjusted statistical comparisons were conducted using Wilcoxon signed-rank test. Adjusted results were estimated using generalized estimating equations controlling for demographics and baseline (6 months before index date) clinical characteristics. There were 1,885 matched pairs (mean age 58.9 years, 52.6% female). Patients with TE had higher proportions of comorbidities versus patients without TE (Charlson comorbidity index [1.2 v 0.5]; cardiovascular diseases [61.3% v 47.6%]; both p<0.0001). In both unadjusted and adjusted results, patients with TE had higher healthcare costs versus patients without TE (adjusted total cost difference: 30-day $20,784; 1-year $44,630; both p<0.0001). Total cost differences were largely driven by differences in inpatient costs. Unadjusted TE-related healthcare costs were $20,036 and $25,311 in the 30-day and 1-year periods, respectively. Among adults with IBD, TE are associated with substantial economic burden. TE risk reduction in patients with IBD could lead to lower economic burden and better patient outcomes.
