•SLR reliability is at least fair when the test provokes lower limb symptoms or pain.•Adding structural differentiation increases SLR reliability to at least a moderate level.•Confidence in estimates of SLR and crossed SLR reliability is moderate to very low.•Large errors occur when different clinicians measure a patient's SLR range of motion.
