Uso del test del susurro para el cribado de pérdida auditiva en mayores institucionalizados con deterioro cognitivo

La prevalencia de esta deficiencia sensorial aumenta en la poblacion mayor institucionalizada, y el numero de residentes sin ayudas auditivas es elevado. A su vez, muchos gerontes que viven en residencias presentan deterioro cognitivo. El test del susurro es una prueba valida para el cribado de deficit auditivo que se utiliza en Atencion Primaria. Al aplicar el test en mayores residentes con deterioro cognitivo hemos advertido que fallan especialmente en la repeticion de letras. Por ello, hemos modificado el procedimiento habitual de la prueba y empleado frases familiares como material de testeo, partiendo del supuesto de que son mas facilmente reconocibles. El objetivo es establecer si esta nueva tecnica puede ser utilizada para el cribado de perdida auditiva en personas con deterioro cognitivo leve-moderado. En la investigacion han participado 62 residentes (MEC > 14). Los primeros resultados revelan que la prueba de voz presenta alta sensibilidad y especificidad, asi como elevados valores predictivos positivo y negativo. La nueva tecnica propuesta podria ser utilizada como herramienta de cribado auditivo en una poblacion similar a la analizada. EnglishRecent studies establish association between hearing loss and cognitive function and evidence exist that hearing damage can exacerbate the negative impact of cognitive impairment in quality of life. Prevalence of hearing loss increases among elderly people living in nursing homes, but the number of residents without hearing aids is high. Whisper voice test is a valid test for screening for hearing impairment. It is frequently used in Primary Care. We applied the voice test in older residents with cognitive impairment and we observed that these subjects failed the repetition of letters. Consequently, we decided to modify the habitual test procedure and employ familiar phrases as screening sound stimulus, on the assumption that they are more easily recognizable. The objective of our study is to establish whether this new technique can be used to screen for hearing loss in older people with mild-moderate cognitive impairment. 62 older residents (mini mental scores > 14) participated in this research. First results reveal that the voice test has high sensitivity and specificity, as well as elevated positive and negative predictive values. The proposed technique could be used as hearing screening tool in a population with similar characteristics.
