Kidneys undergo structural as well as functional aging. Imaging and microscopic exams show alterations that manifest as a decline in glo merular filtration rate (GFR) over time. As a GFR < 60 ml/min/1,73m during more than three months is sufficient to diagnose chronic kidney disease (CKD), a large proportion of elderly fall into this category. However, morphological, clinical and epidemiological data show that the decline in GFR with age is not per se associated with adverse consequences. An age-adapted definition of CKD would allow managing patients on an individual prognostic basis rather than on an arbitrary biological construct.
