•Progressive multifocal leukoencephalopathy (PML) with immune reconstitution inflammatory syndrome (IRIS) in the setting of tofacitinib therapy has not been described in the medical literature.•This is the first case describing a radiologic presentation of PML after chronic use of tofacitinib, with IRIS following cessation of therapy.
