•Prenatal phenobarbitol monotherapy was associated with reduced full-scale and verbal IQ.•Verbal and full-scale IQ lower in phenobarbitol-exposed than phenytoin-exposed.•Phenytoin and carbamazepine not associated with decreases in child performance.•Avoid gestational monotherapy with phenobarbital when alternatives are available.
