•The post-birth period is the highest lifetime risk for bipolar disorder onset.•Score ≥7 on our screen for postpartum bipolar disorder=sensitivity=0.52, specificity=0.82.•Comorbidity ratio bipolar/unipolar Depression: 2.5 PTSD, 2.0 OCD, 2.3 panic disorder.•Nearly half of postpartum women with bipolar disorder reported physical/sexual abuse.
