Cognitive status in a 62-year-old male with lifelong temporal lobe epilepsy and multiple comorbidities

•Presented here is a 62-year-old gentleman with lifelong focal epilepsy.•His cognitive presentation is complicated by developmental, familial, psychiatric, and imaging factors.•Despite these issues his cognition remains remarkably intact.•Understanding his cognitive presentation requires consideration of both risk and resilience factors.
