•PEH are more likely than non-PEH to enter treatment for methamphetamine or heroin use.•PEH report more substance use, mental illness, and criminal justice involvement than non-PEH.•PEH receive more residential care than non-PEH, less intensive outpatient treatment.•PEH have lower rates of treatment retention and successful discharge than non-PEH•Providing more evidence-based care and housing options can improve outcomes for PEH.
