P7‐16: Clinical utility of the electrocardiographic P‐wave axis in patients with chronic obstructive pulmonary disease

Background and Aims: The development of system dysfunction of endothelium in patients with chronic obstructive pulmonary disease (COPD) launches many pathogenetic cascades, it contributes to the defeat of other organs and systems. The aim of the study is to study of the level of endothelin-1 (ET-1), homocysteine (HС) and Willebrand factor (WF) in patients of the COPD of the low and high risk of exacerbations based. Material and Methods: 114 patients were examined (mainly men) with COPD GOLD II-III aged 45-65. Patients were divided into 2 groups: group 1 (n=45) included patients with COPD of the low risk of exacerbations (without exacerbations or 1 which did not lead to hospitalization during the preceding 12 months), group 2 (n=69) included patients with COPD of high risk of exacerbations (≥2 or 1 exacerbation led to hospitalization during the previous 12 months). ET-1, HС and WF were determined in serum by enzyme linked immunosorbent assay. Statistica 10.0 software was used by for statistical processing. This study was done in accordance with the Declaration of Helsinki. Ethics Committee of the Amur State Medical Academy approved this human study. Results and Conclusions: In patients of groups 2 levels of ET-1 and HС were significantly higher than in the group 1 (p<0,05 and p<0,01 respectively). Differences on the concentration of WF are not reliable. The correlation was detected between the level of ET-1 and the exacerbation rate in group 2 (R=0,41, p <0,05). Thus frequent exacerbations of COPD are associated with more pronounced endotheliopathy.
