•The benefits of bloodletting for pneumonia in the nineteenth century were hotly debated.•Pneumonia case fatality, recovery times and iron losses are here calculated from available historical data.•Early bloodletting and higher iron losses were associated with shorter recovery times, despite higher average case fatality with bloodletting.
