•We select patients to receive advanced DBS systems based on clinical features.•54.9% of those implanted with directional leads were using the advanced features.•40 leads (40%) used directional steering (9%), short pulse width (18%) or both (13%).•Advanced features were used by 92% of Vim patients, 44% of STN, and 40% of GPi.
