Iliopsoas abscess is a relatively uncommon condition that can be of primary or secondary origin (gastrointestinal diseases). The classic triad of flank pain, fever and limitation of hip movement is seldom clearly seen. Delayed diagnosis can lead to dramatic complications, such as spondylodiscitis, osteomyelitis and sacroileitis. Computed-tomography (CT) scan and Magnetic Resonance Imaging are very helpful diagnostic tools. Management of these abscesses is complex, thus including medical, infectious and nutritional aspects. The Authors report a case of combined percutaneous and laparotomic approach for the drainage of a large left iliopsoas abscess caused by a left colonic involvement of Crohn's disease. They discuss medical and operative management of iliopsoas abscesses, stressing the importance of the timing and of a multidisciplinary approach to the patient.
