•An observational cohort study of patients with epilepsy.•Participants either used CBD products (n = 280) or did not (n = 138) at baseline.•Cannabidiol Users had higher quality of life and lower psychiatric symptom severity.•Cannabidiol Users had better epilepsy medication tolerability.
