•Low molecular weight heparin-induced thrombocytosis is a rare adverse drug reaction.•Clinicians should anticipate low molecular weight induced thrombocytosis when other causes of thrombocytosis are excluded.•Unfractionated heparin may be a safe therapeutic alternative in case of low molecular weight induced thrombocytosis.
