Infection à COVID 19 et Diabète à l’Hôpital du Mali (Bamako)

ABSTRACT Objective. In December 2019, a new coronavirus was identified in Wuhan, Some scientific articles (Chinese), suggested that diabetes was one of the most common comorbidities in affected patients. COVID-19 can affect both types of diabetes (1 and 2) without a difference in risk. In Mali, the lack of a study on this subject encourageddd us to carry out this study. Population and methods. We conducted a descriptive study, with prospective data collection. Results. Among 512 patients infected with covid 19 collected, 56 were diabetic (11%). The sex ratio was 1.24. The average age was 59.02 years old. High blood pressure and overweight were associated comorbidities, respectively, 43% and 27% of patients. Clinical signs were cough (44.7%), fever (17.9%), headache (10.6%), digestive disturbances (5.4%), general fatigue (7.1%), and rhinorrhea (3.6%), however, 19.6% of patients had no symptoms. The COVID management protocol was that of the Mali national program, all patients benefited from it. All the patients were on anti-diabetic treatment, 48.2% of them were on insulin. The outcome was favorable with 94.6% of the patients cured. The death rate was 5.4%. Conclusion. At the date of study, there was no data confirming an increased risk of contracting COVID-19 in patients with diabetes. This requires special attention, however, as diabetes can be associated with severe, critical and fatal forms of COVID RESUME Objectif. La COVID-19 pouvant toucher a la fois les 2 types de diabete (1 et 2) sans difference de risque. Au Mali, l’absence d’etude a ce sujet nous a pousser a mener cette etude. Population et Methodes. Nous avons mene une etude descriptive, avec recueil prospectif des donnees. Resultats. Parmi 512 patients infectes par la COVID 19 colliges, 56 etaient diabetiques (11%). Le sex ratio etait 1,24. L’âge moyen etait de 59,02 ans. L’hypertension arterielle et le surpoids etaient les comorbidites associees respectivement 43% et 27% des patients. Les signes cliniques etaient la toux (44,7%), la fievre (17,9%), les cephalees (10,6%), les troubles digestifs (5,4%), la fatigue generale (7,1%), la rhinorrhee (3,6%). Toutefois, 19,6% des patients n’avaient aucun symptome.  Le protocole de prise en charge de la COVID etait celui du programme national du Mali, tous les patients en ont beneficie. Tous les patients etaient sous traitement antidiabetique, 48,2% d’entre eux etait sous insuline. L’evolution a ete favorable avec 94,6% des patients gueris. Le taux de deces etait de 5,4%. Conclusion. Au moment de notre etude, aucune donnee ne confirmait un risque accru de contracter la COVID-19 chez le patient diabetique. Celui-ci requiert toutefois une attention particuliere, le diabete pouvant etre associe aux formes severes, critiques et fatales de COVID.
