•We proposed e-Health system for multiparametric telemonitoring of minor stroke/TIA patients.•The proposed solution allowed to promptly and remotely identify vital sign alterations at home.•The proposed solution allowed therapy and behavioral intervention adjustments.•The proposed e-Health solution may be highly useful for early post-stroke remote patient management.
