Extended release injectable naltrexone before vs. after release: A randomized trial of opioid addicted persons who are in prison

•Some prisoners are interested in extended-release injectable naltrexone•Prisoners are much more likely to receive it if offered before vs after release; about 23% continued it through month 4.•Among 16 recorded overdoses, none occurred in patients who were receiving XR-NTX.•Identifying ways to improve treatment retention after release is a very important area to study.
