•Our weight-based enoxaparin dosing was effective at achieving anti-Xa levels at goal.•Enoxaparin 40 mg Q12 reached goal anti-Xa levels in weight ≥ 100 kg or BMI ≥ 40 kg/m2.•The risk of sub-prophylactic anti-Xa levels rises 3% for each 1 kg weight increase.
