•Hypothalamic Hamartoma is a rare congenital disease but with great impact on subcortical epilepsy research.•Associated Epilepsy presents with wide variation of clinical spectrum and psychiatric comorbidities.•Clinical presentation is largely affected by fine anatomical patterns and relation with hypothalamus complexes.•Gamma Knife Radiosurgery could treat the different patterns whatever the hypothalamic area involved.
