We were interested to read the letter from Carvalho and colleagues, who applied the EAN consensus recommendation on cerebral monogenic small vessel diseases (cSVD) (1) to their cohort of patients with suspected genetic cSVD, to determine in which ones they should have studied the Notch3 gene according to the EAN recommendations.
