•Association of mortality-risk with alexithymia was tested in 1380 subjects.•Average observation time was 10 years.•Mortality-risk was increased in male, but not female participants.•“Difficulties describing feelings” showed the strongest effect.•The socioeconomic status showed the strongest impact on this association.
