•We reviewed economic research on ventilator-associated pneumonia (VAP) care bundles.•Reviewed studies suggested lower costs following VAP bundle implementation.•However, the quality of studies varied and had a moderate-to-high risk of bias.•Most studies evaluated local interventions and had unclear generalizability.•Multisite experimental studies are required to address existing research gaps.
