•The therapeutic management of subarachnoid hemorrhage (SAH) is complex and often associated with systemic complications.•PRES is a rare but important finding in SAH patients who receive induced hypertension (IH) for delayed cerebral ischemia (DCI).•It is important to be aware of this complication particularly in older patients, but irrespective of sex, initial status, and treatment modality.
