Lymphangiomas are benign lesions deriving from lymphatic vessels malformations. Although frequent in other glands, adrenal location is extremely rare, with nearly 60 cases described in literature. We report a right adrenal lymphangioma detected during hypertension investigation of a 23 years-old women. Through the study, we found a right adrenal lesion with suspicious image characteristics and doubtful clinical behaviour, without hormonal hypersecretion. Right laparoscopic adrenalectomy was performed. Histological exam revealed an adrenal lymphangioma. After surgery, there was an improvement in blood pressure profile.Adrenal lymphangiomas are rare and the diagnosis is histological. Even without a pathophysiological stablished mechanism, blood pressure normalization is described after surgical removal. Further studies are needed to establish specific characteristics to enable preoperative diagnosis of lymphangiomas, currently approached as adrenal incidentalomas.
