Opioid use prior to liver transplant is associated with increased risk of death after transplant

•17% of transplant recipients had an active opioid prescription at the time of transplant.•Patients using opioids at time of liver transplant have 2.4 times the risk of post-transplant death.•Opioid use prior to transplant should be a consideration in questions of allocation.
