•Our scoping review describes the state of the literature of brain oxygen saturation measured during body position changes on infants.•Many of the studies were performed outside of the United States on preterm neonates.•Most reports demonstrated a statistically significant change in brain oxygen saturation in various body positions.•Fluctuations in brain oxygen saturation during activities, such as position changes, may indicate poor cerebrovascular function.•Changes in brain oxygen saturation in high-risk infants may provide insight on brain health, which can help tailor care.
