•Residential SUD treatment providers may face special challenges during COVID-19.•We interviewed directors of 20 residential SUD treatment programs about COVID-19.•COVID-19 threatened program survival, workforce retention, and client outcomes.•COVID-19 also presented opportunities for improving operations and access.•Residential SUD programs need more funding and specialized guidance during COVID-19.
