Providing respectful maternity care in northern Ghana: A mixed-methods study with maternity care providers

•Even among providers knowledgeable about respectful maternity care and who agreed to be observed providing delivery care, disrespect and abuse were present.•Disrespect and abuse were most common during high-stress events, suggesting the need for training in managing emergencies.•Observed patient provider communication was limited, suggesting future initiatives directed at improving communication are warranted.
