•Serum HE4 and CA125 improved on existing methods for risk stratification of endometrioid carcinomas.•Histological proof of lymph node status was a strength in this study.•Exclusion of lymphadenectomy in low grade endometrioid would have missed LNM in 8.6% of grade 1/2 carcinomas in this study.
