•Up to 40% of all CSVD patients show a pattern of mixed lobar and deep hemorrhages.•Mixed patients were more similar to HA regarding risk profile, CSVD and CSF markers.•cSS presence and CSO-PVS predominance allow classification as “mixed CAA-pattern”•Absence of cSS and CSO-PVS predominance allow classification as “mixed HA-pattern”.•“Mixed CAA-pattern” is more alike CAA in CSVD, CSF and genetic markers.
