Biomarker of the Myocardial Infarction with Non-Obstructive Coronary Arteries

The acute myocardial infarction with non-obstructed coronary arteries (MINOCA) is characterized by the absence of coronary stenosis >= 50% in coronary angiography. The data available on MINOCA is limited due to the heterogeneous definition of the disease and the mostly unresolved etiology in most studies. MINOCA has a frequency of 5.8-11.2% and occurs approximately 2-3 years before classical myocardial infarction. Based on the current body of literature, no conclusive statements can be made about the clinical profile of the patients. Cardiac magnetic resonance imaging is the diagnostic standard and able to clarify the genesis in approx. 75% of cases. The mortality of MINOCA is lower than that of classical myocardial infarction, with affected persons dying primarily from non-cardiovascular causes. Nevertheless, the troponin concentration has prognostic value. For ACE inhibitors, AT1 receptor antagonists and statins, a mortality-lowering effect is suspected.
