• Cerebral thrombotic event can be an important complication of iMCD though it is rarely reported before. • High levels of pro-inflammatory cytokines, hypercoagulative state triggered by hyperfibrinogenemia were observed in iMCD patients. • Angiogenesis within the adventitia of vessel walls induced by VEGF plays a role in chronic inflammation of cerebral vessel walls in iMCD. • The above risk factors increase the susceptibility for ischemic stroke or TIA in iMCD patients. • Further investigation and observation are needed for the underlying mechanism of this rare condition.
